Two medicines, no direct trial
Tesamorelin vs Sermorelin: how they differ
Both medicines start growth hormone release. Their size and study records differ, but no patient trial has compared them head to head.
How tesamorelin differs from the older shot
Chemists made tesamorelin to last longer than a natural brain message. The older medicine, sermorelin, is a shorter chain. Both act on your growth gland. Both tell it to release natural growth hormone. Sermorelin has 29 amino acids, the building blocks of protein. Tesamorelin keeps the full chain and adds a small cap. The cap protects it from enzymes, blood proteins that break medicines apart [5].
That cap helps tesamorelin last longer in blood.
Tesamorelin is approved for one HIV-related belly-fat problem. No patient trial has compared the two medicines head to head.
Their designs differ. No direct study ranks one above the other.
How chain length changes what tesamorelin does
The brain's natural growth message has forty-four protein building blocks. Tesamorelin copies all forty-four. It adds a small cap at one end. That cap keeps blood enzymes from cutting the chain apart [5].
Sermorelin contains only 29 amino acids, the building blocks of protein. It is a shorter chain from the natural message. That shorter chain can start growth hormone release. It has no protective end cap. Blood enzymes can therefore break it down within minutes.
Tesamorelin has the protective cap. In a study, volunteers took 2 mg each day for two weeks [4]. IGF-1, a liver-made growth protein, rose by 181 micrograms per liter. Blood tests showed that the medicine started waves of growth hormone [4].
The end cap helps tesamorelin keep working after the shot.

How the cap protects tesamorelin in the blood
Blood enzymes break natural hormone messages apart within minutes. Enzymes are proteins that cut apart other body chemicals. They can remove the first links of an unprotected chain. That stops the message from doing its work [5].
Chemists added a small cap to the starting end of tesamorelin. The cap blocks those enzymes from grabbing the chain. Tesamorelin therefore lasts longer than an unprotected medicine such as sermorelin.
One small cap made the natural message useful as a daily shot.
What the studies compare and what they don't
The two medicines do not have matching study records.
In Phase 3, tesamorelin reached the last large patient tests. The trials measured organ fat among HIV patients. That single use won approval during 2010. Large patient studies form the main record [1][2]. Separate work checked liver fat [6] and safety in diabetes [7]. A 2026 review brought five trials together [3]. Organ fat fell while IGF-1 rose [1]. This growth protein is made by the liver. The fat came back after shots ended [2].
Doctors once used sermorelin to test how a child's growth gland worked. It also had a role in treating slow childhood growth. No approval covers adult belly fat. No patient study placed both medicines in the same comparison. So a claim that one cuts more fat is only a guess. Changes in IGF-1 don't settle the question either.
Read each medicine on its own proof. A sales claim can't stand in for the missing comparison. If you are weighing either drug, ask your doctor which medical use and patient group the evidence covers.
